Kreider Services, KSB Hospital, Lee County Special Education Association, Ogle County Educational Cooperative, Sinnissippi Centers, Easter Seals, Parent Mentor Project and the Lee County Health Department have been working on a plan to coordinate and improve services to families who have children with an ASD.
Your input will help us better understand and realize your needs and concerns regarding respite care. We ask that you provide your zip code for county identification. We do not need any other identifying information. This is an anonymous survey.
Funding for this survey has been provided through a Rural Health Network (RHN) Planning Grant from Health Resources Services Administration. The RHN grant covers four counties: Lee, Ogle, Carroll and Whiteside.
